Kể từ khi Hướng dẫn năm 2018 của Hiệp hội Tim mạch châu Âu/Hiệp hội Tăng huyết áp châu Âu (ESC/ESH) về quản lý tăng huyết áp động mạch được công bố, nhiều nghiên cứu chất lượng cao, bao gồm các thử nghiệm ngẫu nhiên có đối chứng giả thủ thuật (sham-controlled) về triệt thần kinh giao cảm động mạch thận bằng ống thông (renal denervation – RDN), đã chứng minh hiệu quả hạ huyết áp cũng như tính an toàn của kỹ thuật RDN bằng năng lượng sóng cao tần và siêu âm trên nhiều nhóm bệnh nhân tăng huyết áp, bao gồm cả tăng huyết áp kháng trị.

Hiện nay, RDN được đề xuất là một biện pháp điều trị bổ sung ở bệnh nhân tăng huyết áp kháng trị chưa được kiểm soát, được xác nhận bằng đo huyết áp lưu động, mặc dù đã tối ưu hóa thay đổi lối sống và điều trị bằng thuốc. RDN cũng có thể được cân nhắc ở những bệnh nhân không dung nạp điều trị thuốc hạ huyết áp lâu dài.

Quá trình ra quyết định đồng thuận giữa bác sĩ và người bệnh (shared decision-making) đóng vai trò then chốt và cần có sự tham gia của người bệnh đã được tư vấn đầy đủ về lợi ích cũng như những hạn chế của thủ thuật. Việc chỉ định RDN cần dựa trên nguy cơ tim mạch toàn thể, đồng thời xem xét tổn thương cơ quan đích do tăng huyết áp và/hoặc các biến chứng tim mạch.

Việc đánh giá chỉ định và thực hiện RDN nên được tiến hành bởi nhóm đa chuyên khoa về tăng huyết áp, bao gồm các chuyên gia tăng huyết áp và bác sĩ can thiệp. Bác sĩ thực hiện thủ thuật cần có kinh nghiệm về can thiệp động mạch thận và được đào tạo chuyên sâu về kỹ thuật RDN. Các trung tâm triển khai kỹ thuật này phải có đầy đủ nguồn lực và chuyên môn để xử trí các biến chứng có thể xảy ra.

Trong tương lai, cần có thêm các nghiên cứu nhằm giải quyết những vấn đề còn bỏ ngỏ, đồng thời đánh giá ảnh hưởng của việc hạ huyết áp bằng RDN đối với các biến cố lâm sàng và mở rộng các chỉ định tiềm năng ngoài điều trị tăng huyết áp.

Bài tổng quan trình bày lịch sử phát triển của kỹ thuật RDN, sự mở rộng chỉ định điều trị, cùng các bằng chứng lâm sàng tích lũy từ nghiên cứu SYMPLICITY HTN-1 đến các cập nhật năm 2023.
