Đánh giá sinh lý bệnh động mạch vành: quá khứ, hiện tại và tương lai

Tóm tắt

Tình trạng thiếu máu cục bộ cơ tim là yếu tố tiên lượng quan trọng ở bệnh nhân bệnh động mạch vành. Phân suất dự trữ lưu lượng mạch vành (FFR) và những thông số không cần tình trạng giãn mạch tối đa (sung huyết tối đa) là những phương pháp đánh giá xâm lấn có sử dụng dây dẫn gắn cảm biến áp lực để thăm dò sinh lý lưu lượng tuần hoàn vành (sinh lý tuần hoàn vành) để phát hiện và xác định tình trạng thiếu máu cơ tim do bệnh động mạch vành. Một số nghiên cứu ngẫu nhiên và các nghiên cứu sổ bộ với số lượng bệnh nhân lớn đã chứng minh lợi ích của việc đo các chỉ số sinh lý tuần hoàn vành ở những bệnh nhân mắc bệnh động mạch vành. Ngoài ra, sự phát triển gần đây của các kỹ thuật hình ảnh và thuật toán đã cho phép việc đánh giá sinh lý tuần hoàn mạch vành dựa trên hình ảnh, và tích hợp những thông tin hình ảnh cùng với sinh lý từ các dữ liệu đánh giá xâm lấn cũng như là không xâm lấn. Trong bài báo này, chúng tôi xem xét việc đánh giá sinh lý tuần hoàn vành trong quá khứ, hiện tại cũng như triển vọng trong tương lai.
