Reverse Slider Technique For Hostile Neck Infrarenal Abdominal Aortic Aneurysm Case report

Abstract

Endovascular aneurysm repair (EVAR) is increasingly supported by evidence as a viable alternative to open surgical repair for abdominal aortic aneurysms (AAA). Among the advanced techniques developed to address challenging proximal neck anatomies—such as short, reverse-tapered, or severely angulated necks - is the reverse slider technique. This technique involves two key steps: (1) partial deployment of one to two stent graft, followed by (2) the retrograde retraction of the outer sheath and partially release the bare stent segment of the stent graft above the renal arteries. This maneuver can be repeated two to three times as necessary. The reverse slider technique allows for precise stent graft positioning, enhancing sealing at the proximal neck and thereby minimizing the risk of type IA endoleak. The procedure utilizes the Endurant stent graft system(Medtronic, Santa Rosa, California, USA). We report a successfully treated case of AAA with a short, reverse-tapered, and calcified neck using this technique.
