Abstract
Objective: To describe clinical characteristics and evaluate pregnancy outcomes among women with heart disease from 22 weeks of gestation at Thanh Hoa Obstetrics and Gynecology Hospital during 2023-2025. Methods: A retrospective cross-sectional study was conducted on 412 pregnant women diagnosed with heart disease and delivered at Thanh Hoa Obstetrics and Gynecology Hospital between January 2023 and December 2025. Data regarding maternal characteristics, cardiac disorders, cardiovascular complications, pregnancy outcomes and delivery methods were collected from electronic medical records and analyzed using SPSS version 26.0. Results: Mean maternal age was 29.4 ± 5.3 years, with women aged 20-34 years accounting for 82.5% of cases. Congenital heart disease represented 45.6% and acquired heart disease accounted for 42.7%; 81.6% of women had been diagnosed before pregnancy. The most common manifestations were abnormal cardiac murmurs (60.2%) and dyspnea (23.1%). Cardiovascular complications occurred in 28.6% of patients, including arrhythmias (14.1%) and heart failure (13.1%). Preterm birth occurred in 8.5%, fetal growth restriction in 5.8% and stillbirth in 1.0%. Cesarean section was performed in 93.4% of pregnancies, with cardiovascular indications accounting for 58.4%. No maternal deaths were recorded. Conclusions: Most women with heart disease achieved favorable pregnancy outcomes under multidisciplinary management. Arrhythmia and heart failure were the most common cardiovascular complications. Cesarean section was the predominant mode of delivery.
