Assessment of coronary physiology: past, present and future

Abstract

The presence of myocardial ischemia is an important prognostic factor in patients with ischemic heart disease. Fractional flow reserve (FFR) and hyperemia-free indexes are used as invasive methods to detect and identify the ischemia-causing coronary disease. Several randomized trials and large registries proved the benefit of measuring physiologic-indexes in patients with coronary artery disease. In addition, recent development ofimaging and computational technologies has enabled image-based physiologic assessment and integration of imaging and physiologic information from invasive and non-invasive assessment data. In this manuscript, we review the history and current status of coronary physiologic assessment and its future perspectives.
